To add your name to our waiting list and to apply for admission to Grace Christian Home and/or Connaught Home, download and print this four-page form.  Once we receive the completed form by mail or fax, we will add your name to our waiting list and will contact you to determine the specifics of your needs.  There is no charge to add your name to our wait list. You may apply for residency in one of our Homes, or in both, as you wish.  When we have a room available, we contact those on our admissions list in priority order. 

Medical form (18 KB PDF)

Once you have applied for residency, take this 4-page form to your doctor at your next appointment.  This form is not required until you expect to take up residency within the next 12 months.
